Hi doctors,\nMy name is David, and I am asking on behalf of my father. He is 70 years old, and has had liver cirrhosis for many decades. Currently we are in the United States, but unable to find proper care and diagnosis. He has been taking medicine for many years to no avail. Recently I have found out about your great organization, and wondering if we can visit Taiwan to consult with your researchers, and for diagnosis and treatment. Once a patient is diagnosed with liver cirrhosis (the diagnosis can be made either by clinical diagnosis, liver biopsy or non-invasive fibrosis assessment), the next step we will perform is a comprehensive evaluation of the patients' liver function. If the liver function is adequate (we call it "compensated," meaning no ascites, jaundice, encephalopathy, esophageal variceal bleeding...), close follow-up of liver function and surveillance of hepatocellular carcinoma are needed. However, there is no approved drug that can directly reverse the status of cirrhosis (or fibrosis). What we can do is to find out the cause of the patients' cirrhosis. The most common causes include hepatitis B (usually in Asians), hepatitis C, alcoholism, and fatty liver. For hepatitis B and C, we now have very potent antiviral drugs. For alcoholism, stopping drinking is the only method. For fatty liver, diet control and exercise for body weight reduction are the only way. (There are some drugs that can reduce body weight, but they are still in clinical trials regarding their effects on fatty liver-related cirrhosis). If the cirrhotic patient develops hepatic decompensation (ascites, jaundice, encephalopathy...), the management will be controlling and preventing the cirrhotic complications. If that occurs, liver transplant will also be suggested.

From your description, we do not have much information regarding your father's current status, and can just give you the suggestions as above. I would also suggest you to talk with your father's doctor regarding your concern and discuss whether to refer your father to a hepatology specialist if necessary.

請問我的親戚16歲昨天發生車禍雙手骨折,抽血發現他GOT 654,沒有B、C肝,生命徵象穩定,右腹部有輕微疼痛,請問是因為肝臟有破裂才造成他數值這麼高嗎?還是只是車禍造成他肝細胞受損,暫時性的升高?
